What billing or insurance information will I receive?
Dr. Cleary does not accept nor file insurance. She is not a Medicaid Provider. You must pay in full at the time of your visit. We will provide you a superbill with the necessary information for you to file your claim.

New Patient Appointment - $325
Follow Up Appointment (20-30 minutes) - $150
Follow Up Appointment (45-60 minutes) - $275

These can be emailed/mailed to you upon your request or you may download them. Click the link to print out the paperwork. Once completed you may fax or email them back to us.
If you are seeking an appointment for your child, please complete the Child/Adolescent Paperwork. If you have specific concerns that your child may have ADHD or other school related issues, please also complete the Teacher and Parent Questionnaire/ADHD forms.
Adult patients are typically only seen by Dr. Cleary if she worked with them as a child or adolescent. If you are an adult and wish to schedule an appointment with Dr. Cleary, please complete the Adult Paperwork.
